Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator in Jackson County, Ohio. The role focuses on planning and organizing a team to support youth at home, school, and in the community with the goal of keeping families together and reducing hospitalizations.

You will lead family and youth teams, coordinate services, document activities, and travel within the service area while adhering to OhioRISE standards and HIPAA requirements.

Qualifications

• Experience serving children and families in behavioral health, child welfare, or related public services.
• Two years with an associate or bachelor's degree; or one year with a master's degree or higher.
• Knowledge of Hi-Fidelity Wraparound is preferred (certification provided at time of employment).
• Two years in a care coordination or coordinated supportive services role preferred.

Responsibilities

• Lead and facilitate team meetings for the young person and family.
• Provide care coordination in home, school, or community settings.
• Partner with family, supports, and service providers to identify needs and actions.
• Maintain documentation to access and sustain OhioRISE benefits.
• Travel within the assigned service area and ensure HIPAA compliance.
• Meet quality and fidelity requirements per ISBH OhioRISE standards.
• Attend trainings and maintain required certifications.
• Safely handle confidential information and bill Medicaid without restrictions.
