Job summary

Integrated Services for Behavioral Health (ISBH) in Athens, OH, is seeking a Care Coordinator to organize and lead a youth-focused care team under OhioRISE. The role supports families at home, school, and in the community, ensuring voice and choice in coordinated services.

Responsibilities include planning meetings, coordinating with service providers, maintaining documentation, and meeting HIPAA and fidelity standards.

Qualifications

• Experience providing services to children and families in behavioral health or related fields.
• Hi-Fidelity Wraparound knowledge preferred (Certification provided at time of employment).
• Two years in a care coordination or similar role with high efficiency and attention to detail is preferred.
• Experience with autism spectrum disorders and developmental disabilities is preferred.
• Ability to communicate effectively, both verbally and in writing.

Responsibilities

• Develop, lead, and facilitate team meetings for the young person and identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Collaborate with families, natural supports, and service providers to identify needs and services.
• Maintain documentation to access and maintain OhioRISE benefits.
• Travel within assigned service area and complete trainings and certifications.
