Job summary

Integrated Services for Behavioral Health (ISBH) in Jackson, OH is seeking a Care Coordinator to lead child and family teams, plan services at home, school, and within the community, and support families to stay together and avoid hospitalizations.

You will collaborate with social service providers, partner with families and supports, maintain required documentation, and ensure the youth and family have a voice and choice in every service.

Qualifications

• Experience providing services to children and families in behavioral health or related fields.
• Hi-Fidelity Wraparound knowledge preferred (Certification provided at time of employment).
• Two years in a coordinated supportive services or care coordination role.
• Experience with autism spectrum disorders and developmental disabilities preferred.
• Ability to bill Medicaid without restrictions.

Responsibilities

• Lead and facilitate team meetings for the young person and identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Partner with the family, supports, and service providers to identify needs and services.
• Maintain required documentation to access and maintain OhioRISE benefits.
• Meet quality and fidelity requirements per ISBH OhioRISE standards.
• Travel within the assigned service area and participate in trainings.
