Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to essential services in home, community, and office settings.

You will coordinate care with the treatment team and supervise staff, participate in weekly meetings, and ensure regulatory compliance while actively recruiting referrals to sustain a full caseload.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years’ experience working with children and/or families; internships and volunteer work count.

Responsibilities

• Develop individualized treatment plans in collaboration with the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building per the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as liaison with other professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions, treatment plans, and progress notes per policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.

Education

Bachelor’s degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field
