Job summary

Pinnacle Family Services, part of Health Connect America, looks for a Family Centered Treatment (FCT) specialist to support children and families in North Carolina. You will develop individualized service plans, coordinate with schools and medical teams, and guide families through the four phases of the FCT model to promote sustainable well‑being.

This community-based role requires NC licensure or certification, a relevant degree, and a commitment to trauma‑informed care, with a flexible

Qualifications

• Master's in human services field with 1 year of experience or
• Bachelor's in human services field with 2 years of experience or
• Bachelor's in non-human services field with 4 years of experience
• Current NC license or board-issued certification in a human services profession
• Experience serving children/adolescents with a primary mental health diagnosis
• Valid driver’s license and access to private transportation

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are.
• Guide families through the four phases of the FCT model and help set goals and build skills.
• Coordinate with medical, behavioral health, school, and community partners to ensure seamless care
• Develop and refine individualized Service Plans in partnership with families
• Connect families to community resources, referrals, and aftercare supports
• Maintain timely documentation in the Electronic Health Record reflecting quality of care
• Participate in Quality Assurance activities to strengthen programs
