Job summary

Pinnacle Family Services, part of Health Connect America, seeks a Family Centered Treatment (FCT) professional to support children and families in home and community settings. You will work with the Treatment Team to implement personalized plans and coordinate with partners across medical, behavioral health, school, and community resources.

FCT Certification is required within one year of hire. The role promises flexible, community-based work with ongoing professional growth and a strong focus

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104. One of the following
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis, and may be accumulated before or after degree completion.
• Must have a valid driver’s license and access to a fully insured, private form of transportation

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are — literally and figuratively
• Guide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well-being
• Serve as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care
• Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve
• Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together
• Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide
• Participate in Quality Assurance activities that help make our programs stronger for every family we serve
