Health Connect America is seeking a Family Centered Treatment Specialist to deliver FCT services for children and families, working in home, community, and office settings. This role requires FCT certification and active participation in plan development and documentation.

You will coordinate with the Treatment Team, provide psychoeducation, and support families in achieving goals within a flexible schedule, including on-call coverage where needed.

Qualifications

• Minimum of a Bachelor’s degree in human services or related field.
• FCT Certification preferred or required in multiple states.
• Clinical documentation and service planning responsibilities are required.

Responsabilités

• Provide Family Centered Treatment (FCT) services in clients’ homes, communities, and offices.
• Complete FCT checkoffs and trainings within deadlines.
• Develop and coordinate Service Plans with Treatment Team and ensure timely documentation.
