Pinnacle Family Services, part of Health Connect America, Inc., seeks a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities across North Carolina. Join a team dedicated to trauma-informed, client-centered care for children and families.

Role requires FCT certification within one year and a valid driver's license. You will work with schools, medical and behavioral health partners to coordinate care and develop individualized service plans for

Qualifikationen

• NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Master's in human services with 1 year experience.
• Bachelor's in human services with 2 years experience; or non-human services with 4 years.
• Current NC license or board certification in a human services profession.
• Experience must be full-time and supervised with children/adolescents with a primary mental health diagnosis.
• Valid driver’s license and access to insured transportation.

Aufgaben

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide children and families through the four phases of the FCT model and set goals.
• Serve as central point of coordination for each family's Treatment Team.
• Develop and refine individualized Service Plans with families.
