Pinnacle Family Services, part of Health Connect America, is hiring to deliver Family Centered Treatment in home/community settings across NC. You will guide families through the four FCT phases, coordinating with medical, behavioral health, school, and community partners for holistic care.

The role requires FCT certification within one year and a strong commitment to children and families. A valid NC license or board certification is essential and a driver’s license is required.

Qualifikationen

• Master's in human services field with 1 year of experience.
• Bachelor's in human services field with 2 years experience.
• Bachelor's in non-human services field with 4 years experience.
• Current NC license or board certification in a human services profession.
• Full-time, supervised experience with children/adolescents with a primary mental health diagnosis.
• Must have a valid driver's license and access to insured transportation.

Aufgaben

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide children and families through the four phases of the FCT model and set goals.
• Serve as a central point of coordination for the family's Treatment Team.
• Develop, implement, and refine individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely documentation in the Electronic Health Record reflecting quality of care.
• Participate in Quality Assurance activities to strengthen programs.
