Health Connect America is seeking a Family Centered Treatment Specialist to deliver FCT services for children, adolescents and families. The role emphasizes fidelity to the FCT model, collaborative planning, and responsive service delivery in home and community settings.

You will work with a treatment team, manage service plans, complete documentation in the EHR, and participate in QA/QI activities. A Bachelor's degree and 3+ years of related experience are required.

Qualifications

• Bachelor's Degree in human services or related field.
• Three years’ experience working with youth or families.

Responsabilités

• Provide Family Centered Treatment (FCT) services to children, adolescents and families.
• Ensure fidelity to the FCT model and service definitions.
• Complete required trainings and checkoffs within deadlines.
• Develop and coordinate Service Plans with the treatment team.
• Maintain clinical records and ensure timely documentation in the EHR.
• Provide 24/7 on-call coverage and flexible scheduling to support families.
• Collaborate with medical and non-medical providers for coordinated care.
• Monitor progress and participate in QA/QI activities.
