Job summary

Pinnacle Family Services in North Carolina seeks a Family Centered Treatment clinician to work in home and community settings, guiding families through the FCT four-phase model and coordinating care with medical, behavioral health and school partners.

You will develop Service Plans with families, connect them to resources, and maintain accurate EHR documentation. FCT Certification is required within one year; 24/7 on-call support is part of the role.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• One of: Master's in human services field + 1 year experience, or Bachelor's in human services field + 2 years, or Bachelor's in non-human services field + 4 years.
• Must have a valid driver’s license and access to private transportation.
• Full-time and supervised experience with children/adolescents with primary mental health diagnosis.

Responsibilities

• Deliver FCT services primarily in home and community settings, meeting families where they are.
• Guide families through the four phases of the FCT model and help set goals and build skills.
• Coordinate with medical, behavioral health, school and community partners for whole-person care.
• Develop and refine individualized Service Plans with families.
• Connect families to community resources and aftercare supports.
• Maintain timely documentation in the Electronic Health Record and participate in QA activities.

Current NC license or board-issued certification in a human services profession
