Job summary

Pinnacle Family Services, under Health Connect America, seeks a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in home and community settings across North Carolina. You will work with children, adolescents, and families within a structured treatment model designed to create lasting change.

Qualifications

• Must meet NC Qualified Professional requirements per 10A NCAC 27G .0104. One of the following
• Current NC license (including provisional) or board-issued certification in a human services profession
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are.
• Guide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well-being
• Serve as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners
• Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve
• Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together
• Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide
• Participate in Quality Assurance activities that help make our programs stronger for every family we serve
