Job summary

Pinnacle Family Services, part of Health Connect America, seeks a dedicated clinician to deliver Family Centered Treatment in home and community settings across North Carolina. You will guide families through the FCT model, coordinate with a multidisciplinary team, and help families build sustainable well‑being.

The role emphasizes trauma-informed, client-centered care, with a focus on collaboration, ongoing plan refinement, and access to community resources to extend impact after sessions.

Qualifications

• Must meet NC Qualified Professional requirements per 10A NCAC 27G.0104
• FCT Certification within one year of hire
• Experience with children and families in primary mental health context

Responsibilities

• Deliver Family Centered Treatment services in home/community settings.
• Guide families through the four phases of the FCT model and set goals.
• Coordinate Treatment Team with medical/behavioral health/school partners.
• Develop and adapt individualized Service Plans with families.
• Connect families to community resources and aftercare supports.
• Maintain accurate EHR documentation and reflect quality of care.
