Health Connect America, Inc. is hiring for a Family Centered Treatment clinician to deliver FCT services to children and families in home and community settings. The role requires completing FCT Certification within one year and coordinating with a Treatment Team to develop and execute Service Plans.

Team members provide flexible scheduling, participate in quality initiatives, and work within a comprehensive benefits package including medical, dental, vision, and more.

Qualifications

• Bachelor's Degree in human services or related field is required.
• Two years' experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.
• State-specific credentialing requirements vary by state (Mississippi / NC / OH / VA).
• Mississippi – provisional PCSS/CCSS eligibility; North Carolina – Qualified Professional qualifications; Ohio – LSW or provisional licensure; Virginia – QMHP or VA Board registration.

Responsabilités

• Provide Family Centered Treatment (FCT) services as defined by the model and fidelity requirements.
• FCT Certification is required and must be completed within one year.
• Deliver FCT services to children and families, primarily at home and in the community with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Complete clinical records and FCT documents in the EHR timely; coordinate Service Plans with the Treatment Team.
• Develop and coordinate discharge plans and aftercare needs with community resources.
