Qualifications

• Experience with coordinating services across families, youth, and multiple providers.

Responsibilities

• Provide Family Centered Treatment (FCT) services in home and community settings with a family focus.
• Maintain fidelity to the FCT model and complete required checks and trainings.
• Deliver services to children, adolescents and families; maintain timely documentation in the Electronic Health Record.
• Provide 24/7 on-call coverage for a caseload of families and children with flexible scheduling.
