Health Connect America, Inc. is seeking qualified clinicians to provide Family Centered Treatment (FCT) services across multiple states, with a focus on children, families, and communities.

The role requires completing FCT certification within one year and delivering services in home and community settings with a strong emphasis on family engagement. Minimum education includes a Bachelor's in human services or related field, plus state-specific experience requirements.

Qualifications

• Bachelor's degree in human services or related field is required.
• Two years' experience working with youth or families is required in several states.
• FCT Certification is required within one year of employment to continue providing FCT services.

Responsabilités

• Provide Family Centered Treatment (FCT) Services per the service definition and model fidelity requirements.
• Deliver FCT services primarily in the home and community with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Develop and coordinate Service Plans with Treatment Team and coordinate discharge planning.
• Consult with medical and non-medical providers to coordinate services.
• Ensure timely documentation in the Electronic Health Record.
