Health Connect America is hiring a Family Centered Treatment Specialist to deliver FCT services to children, adolescents, and families primarily in their homes and communities. The role emphasizes collaboration with the Treatment Team, planning, and ongoing documentation.

Ideal candidates hold a Bachelor's degree in human services, have at least two years of experience with youth or families, and will obtain FCT Certification within one year.

Qualifications

• Minimum of a Bachelor's degree in human services or related field.
• Two years’ experience working with youth or families.
• FCT Certification is required and must be completed within one year of employment to continue providing services.

Responsabilités

• Provide Family Centered Treatment (FCT) Services per the service definition and model fidelity requirements.
• Deliver FCT services primarily in the home and community with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Complete Clinical Records and FCT documents in the Electronic Health Record per company standards.
• Develop and coordinate the Service Plan with Treatment Team input and maintain updates as needed.
• Convene the Treatment Team for Service Plan reviews and ensure communication across providers.
• Coordinate discharge planning and link families to aftercare resources.
