Job summary

Health Connect America is seeking a Family Centered Treatment Specialist to deliver FCT services to children and families, primarily in home and community settings, with a family-centered focus.

The role requires coordination with the treatment team, ongoing service planning, and ensuring timely documentation in the EHR system. A strong commitment to evidence-informed care is essential.

Qualifications

• Bachelor’s degree in human services or related field required.
• Minimum of two years’ experience with youth or families.
• FCT Certification preferred; ongoing certification within 1 year.
• State-specific requirements listed: Mississippi, NC, Ohio, Virginia.

Responsibilities

• Provide Family Centered Treatment (FCT) services to children, adolescents and families in home or community settings.
• Complete FCT checkoffs and trainings within designated deadlines and document in EHR.
• Develop, implement and revise the Service Plan with Treatment Team input and monitor progress.
• Coordinate discharge planning and link to community resources for aftercare.

Education

Bachelor's Degree in human services or related field

Two years’ experience working with youth or families

Family Centered Treatment Certification

Mississippi credentialing requirements (PCSS/CCSS)

North Carolina Qualified Professional (QP)

Ohio LSW or provisional licensure

Virginia QMHP eligibility or credential
