Job summary

Health Connect America seeks a Family Centered Treatment provider to deliver FCT services to children, adolescents, and families, primarily in homes and communities. The role focuses on creating and updating Service Plans, coordinating with the treatment team, and maintaining clinical documentation in the EHR.

Responsibilities include on-call coverage, discharge planning, and collaboration with medical and social supports to ensure continuity of care and family-focused outcomes.

Qualifications

• Bachelor’s degree in human services or related field.
• Two years’ experience working with youth or families.
• QMHP eligibility or certification where required by state.

Responsibilities

• Provide Family Centered Treatment (FCT) services as defined by the model.
• Maintain FCT certification within one year of employment.
• Develop and monitor Service Plans with the treatment team; coordinate care.
• Deliver services in home/community settings with family focus.
• Provide 24/7 on-call coverage for caseloads of families and youth.
• Document clinical records and ensure timely entry in EHR.
• Coordinate discharge plans and link to community resources.
• Collaborate with medical and non-medical providers to support client outcomes.
• Participate in quality assurance and improvement activities.

Education
