Zusammenfassung

Pinnacle Family Services, part of Health Connect America, seeks a Family Centered Treatment (FCT) Specialist. You will deliver evidence-based services in children’s homes and communities, guiding families through the FCT model and coordinating with a Treatment Team to ensure holistic care.

The role emphasizes hands-on, relationship-driven work with a flexible schedule and mandatory FCT Certification within one year of hire. Equal opportunity employer.

Qualifikationen

• Meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• One of: Master’s in human services + 1 year experience
• Bachelor’s in human services + 2 years experience
• Bachelor’s in non-human services field + 4 years experience
• Current NC license or board certification in a human services profession
• Experience must be full-time and supervised, serving children/adolescents with a primary mental health diagnosis
• Must have a valid driver’s license and access to private transportation

Aufgaben

• Deliver Family Centered Treatment services in home and community settings.
• Guide families through the four phases of the FCT model.
• Coordinate with medical, behavioral health, school, and community partners.
• Develop, implement and refine individualized Service Plans.
• Connect families to community resources, referrals and aftercare.
• Maintain timely documentation in the EHR.
• Participate in Quality Assurance activities.
