Job summary

Pinnacle Family Services, part of Health Connect America, seeks a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in the homes and communities of children and families. You’ll work hands-on in a structured, relationship‑driven model and guide families through four FCT phases to lasting change.

Expect a flexible, community-based schedule with 24/7 on-call crisis support. You’ll pursue FCT certification within one year and maintain comprehensive EHR documentation to

Qualifications

• Must be NC Qualified Professional (QP) per 10A NCAC 27G .0104 with one of the listed degree/experience combinations.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis.
• Must have a valid driver’s license and access to private transportation.

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings.
• Guide children and families through the four phases of the FCT model and help set goals.
• Coordinate with Treatment Team across medical, behavioral health, school and community partners.
• Develop, implement, and refine individualized Service Plans with families.
• Connect families to community resources, referrals, and aftercare supports.
• Maintain timely documentation in the Electronic Health Record (EHR).
• Participate in Quality Assurance activities to strengthen programs.
