UCFS Healthcare is seeking a Care Coordinator to provide support services to families with high-risk children, addressing behavioral or emotional issues, parenting challenges, and life stressors through comprehensive assessments and service plans. The role includes outreach, care planning, and weekly direct services.

Applicants should have a Bachelor's in Human Services or 5 years of relevant experience, with strong communication and collaboration skills; CPR and Wraparound certifications are

Qualifications

• Bachelor's Degree in Human Services with a minimum of 1 year of experience or training in social services or 5 years of relevant Human Services experience.
• Experience working with children and families with significant behavioral health needs.
• Home visiting experience highly preferred.

Responsibilities

• Conduct initial phone intake with families to determine eligibility.
• Assess family strengths and needs to develop a care plan during home visits.
• Provide weekly face-to-face services including advocacy, referrals, and life skills coaching.
• Coordinate Child Specific Team meetings and implement recommendations.
• Maintain case records and weekly contact reports.
