UCFS Healthcare in Norwich, Connecticut seeks a dedicated Care Coordinator to support families with high-risk children, conducting bio-psycho-social assessments and developing service plans. The role includes outreach, home visits, and coordinating a team to implement care strategies.

You will manage case records, collaborate with community partners, and participate in ongoing trainings to enhance client-centered care. Open 40 hours/week with travel within UCFS service area.

Competenze

• Bachelor's degree in Human Services or related field; 1 year of social services experience or training.
• OR 5 years of relevant Human Services experience.
• Experience with children and families with behavioral health needs.
• Home visiting experience preferred.

Mansioni

• Provide outreach and engagement to clients referred to the program.
• Complete family assessments and develop care plans.
• Provide weekly face-to-face services including support, advocacy and referrals.
• Coordinate multidisciplinary team meetings and follow through with recommendations.
• Maintain client records and weekly case notes and summaries.
• Participate in team meetings and trainings.
