Job summary

United Community & Family Services, Inc. seeks a Care Coordinator to support high-risk families with behavioral health needs in Eastern CT. You will conduct assessments, develop care plans, and coordinate services with a multidisciplinary team.

The role requires a Bachelor’s in Human Services or 5 years of relevant experience, willingness to travel within UCFS’s area, and a client-centered, fast-paced approach. UCFS offers comprehensive benefits and opportunities for WRAP certification.

Qualifications

• Bachelor’s Degree in the Human Service field with 1 year experience or training in social services.
• OR 5 years of relevant Human Services experience.
• Experience working with children and families with significant behavioral health need.
• Home visiting experience highly preferred.

Responsibilities

• Provide outreach and engagement to client upon referral, collecting information to determine eligibility.
• Assess the family using home visits and Ohio scales to develop a care plan.
• Provide weekly face-to-face direct services including support, advocacy and referrals.
• Develop a list of interested parties for Child Specific Team meetings and follow through with recommendations.
• Maintain community relations and participate in external meetings.
• Attend case reviews and trainings and maintain case records.

Education

Bachelor's Degree in Human Service field

5 years of relevant Human Services experience

Home visiting experience

Strong communication and teamwork

Proficiency with Microsoft Outlook
