UCFS Healthcare in Norwich, CT, is hiring a Care Coordinator to support families with high-risk children by developing and implementing bio-psycho-social assessments and service plans.

You will conduct outreach, perform home visits, and provide weekly direct services, including advocacy and parenting support, while coordinating with the Child Specific Team and maintaining case records. This role requires travel and strong teamwork.

Qualifications

• Bachelor's degree in Human Service field with 1+ year experience or 5 years of relevant experience
• Experience working with children and families with significant behavioral health need
• Home visiting experience preferred

Responsabilités

• Provide outreach and engagement to client upon referral by conducting an initial phone intake and determining eligibility
• Complete a family assessment using home visits and Ohio scales to develop a care plan
• Provide weekly direct service hours including advocacy, referrals and parenting support
• Develop and coordinate Child Specific Team meetings and implement recommendations
• Maintain up-to-date case records and participate in supervision and trainings
