Job summary

Redwood Community Services, Inc. is seeking a Care Coordinator to coordinate family-centered, multidisciplinary wraparound teams focused on assessing and addressing the individualized needs of children and their families.

You will identify strengths and challenges, develop tailored care plans, and connect families with appropriate community resources and supports to promote stability and long-term success. The role requires organizing and leading CFT meetings, ensuring consent and information

Qualifications

• Bachelor of Social Work (BSW), or Bachelor's degree (BS), or Master's (MA) in a social science field with at least two years' related experience or training.
• Licensed or Intern with BBS.
• Specialized knowledge of at-risk youth in multi-service systems of care; WRAParound experience is a plus.

Responsibilities

• Coordinate and facilitate family-centered, multidisciplinary wraparound teams.
• Organize and facilitate Child and Family Team (CFT) meetings to develop and revise the Family Care Plan.
• Review billing forms and related documentation for accuracy and compliance with policies.
• Identify and report clinical concerns to program managers and supervisory staff promptly.
• Perform comprehensive strengths, needs, and cultural assessments for care planning.
• Lead wraparound team meetings to integrate professional services with natural supports.
• Ensure collaborative development and revision of service plans including safety planning.
• Facilitate access to formal and informal supports and community resources.
• Support transitions across service levels to adulthood.
