Qualifications

• Bachelor's degree in Human Service field with a minimum of one year experience or training in social services or 5 years of relevant Human Services experience
• Experience working with children and families with significant behavioral health need
• Home visiting experience highly preferred

Responsibilities

• Provide outreach and engagement to clients upon referral by conducting initial phone intake
• Assess family strengths and needs to develop a care plan
• Provide weekly face-to-face direct services including advocacy, referrals, safety planning, and parenting support
• Coordinate with Child Specific Team meetings and follow through with recommendations
• Maintain community relations and participate in external meetings and trainings
• Participate in case reviews and supervise and attend team meetings
• Maintain up-to-date case records including weekly contacts and evaluations
