Job summary

Mental Health Resource Center, Inc. is seeking a FACT Team Leader to administer Florida Assertive Community Treatment services.

This role leads a multidisciplinary clinical team delivering treatment, rehabilitation, and support to adults with serious mental illness, promoting recovery and empowerment in community settings. The position oversees coordination of care, assessments, and recovery planning, with responsibilities for staff supervision, scheduling, and 24/7 on-call coverage.

Qualifications

• Master’s degree in Human Services or a related field is required.

Responsibilities

• Assesses and screens individuals for FACT services and coordinates admissions with the psychiatrist.
• Assigns staff to ITT and designates primary case managers.
• Oversees development of initial and comprehensive assessments and recovery plans.
• Provides clinical supervision and leads treatment planning and recovery meetings.
• Delivers direct clinical, rehabilitation and support services as an active team clinician.
• Coordinates daily team meetings to prioritize services and ensure timely care.
• Provides clinical supervision, crisis support, and on-call coverage (24/7 rotation).
• Monitors risk, trauma, abuse, and neglect and ensures reporting.
• Reviews clinical documentation for accuracy and timeliness.
