Job summary

The David Lawrence Center is seeking a dedicated FACT Case Manager to deliver intensive behavioral health services in community settings. You will work within a multidisciplinary team to promote recovery, reduce hospitalizations, and support adults with serious mental illness and co-occurring conditions.

• Bachelor's degree in a relevant field is required.
• Experience in behavioral health or case management preferred.
• Familiarity with recovery-oriented and trauma-informed care.

Responsibilities

• Serve as primary case manager for assigned caseload on the FACT team.
• Complete assessments and recovery plans with participants and families.
• Provide case management, counseling, crisis intervention, and care coordination.
• Assist participants with housing, benefits, transportation, and daily living activities.
• Coordinate with health, housing, legal, vocational, and community resources.
• Maintain timely, confidential documentation in the electronic health record.
