Job summary

David Lawrence Center is seeking a FACT Case Manager to join a multidisciplinary team delivering intensive behavioral health services in community settings. You will support adults with serious mental illness and co-occurring disorders, focusing on recovery, stability, and independent living through collaborative planning and coordination.

The role requires flexible hours, including evenings and rotating weekends, with strong emphasis on documentation, crisis intervention, and interagency

Qualifications

• Bachelor's degree in social work, psychology, counseling, rehabilitation, human services, criminal justice, or related field.
• Experience in behavioral health, substance use, rehabilitation, community-based, or case management services.
• Knowledge of recovery-oriented care, trauma-informed practices, and co-occurring conditions.
• Experience in crisis intervention, housing coordination, benefits navigation, or vocational support.
• Training in motivational interviewing or related practices; bilingual preferred.

Responsibilities

• Serve as the primary case manager for an assigned caseload within a team-based FACT model.
• Complete assessments, recovery plans, and treatment or crisis plans with participants and families.
• Provide case management, counseling, care coordination, and advocacy in community settings.
• Assist with housing stability, benefits navigation, transportation, and daily living activities.
• Support medication adherence and coordinate with nursing and medical staff.
• Document accurately in the electronic health record and meet regulatory requirements.
