Job summary

Mental Health Resource Center, Inc. seeks a FACT Case Manager to deliver treatment, rehabilitation, and support services within a multi-disciplinary FACT team serving adults with severe mental illness in the community.

Responsibilities include assessments, case management, referrals, and medication management, with most time spent in the community; occasional office work and on-call duties are required. Equal opportunity employer.

Qualifications

• Bachelor's degree in Human Services or a related field is required.
• Previous experience working with adults with severe and persistent mental illness is required.
• Driver requirements include a Florida Driver's License, automobile insurance of $200,000, and no more than eight points on the license.
• Ability to make sound decisions independently daily, including emergencies and life-threatening situations.
• Strong communication skills are essential and interaction with families, caregivers, and providers is expected.

Responsibilities

• Provide treatment, rehabilitation, and support services per the FACT model.
• Maintain accuracy, completeness, and confidentiality of clinical records.
• Conduct mental health status assessments and coordinate referrals.
• Assist with activities of daily living, transportation, and social/interpersonal skills.
• Serve as primary case manager for a group of individuals, promoting recovery and empowerment.
