Job summary

Mental Health Resource Center, Inc. seeks a FACT Case Manager to provide treatment and supportive services to adults with serious mental illness in the community. The role emphasizes recovery, empowerment, self-determination, and coordinated care through a multidisciplinary team.

Qualifications

• Bachelor’s degree in Human Services or related field.
• Experience with adults with severe mental illness.
• Valid Florida driver’s license and auto insurance.
• Ability to work independently and in community settings.
• Strong communication with families, caregivers and providers.

Responsibilities

• Provide treatment, rehabilitation, and support services per FACT model.
• Maintain accuracy and confidentiality of clinical records.
• Conduct mental health status assessments.
• Coordinate referrals to community providers and supports.
• Support activities of daily living, transportation, and social skills.
• Educate about symptoms, medications, and mental health issues.
• Serve as primary case manager for an assigned group and promote recovery.
