Job summary

Haven Home Care in Eastern Ohio seeks a Supervisory LPN to support our expansion into Ashtabula, Columbiana, Mahoning, and Trumbull counties. The role focuses on supervisory visits, client onboarding, and documentation, with pay tied to completed visits and tests rather than hourly wages.

Initial hours are modest as we build our client base; mileage is reimbursed and an iPad with data plan is provided. The position offers flexible scheduling and part-time hours in a growing territory.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and current auto insurance that covers driving for work.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Documentation: Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Supervisory and Open Visits: Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
• Pre-Employment Testing: Conduct skills tests and written tests for Home Health Aide candidates.
