Job summary

Haven Home Care is expanding its Eastern Ohio presence and seeks an LPN to supervise Home Health Aides and complete open and supervisory visits across Ashtabula, Columbiana, Mahoning, and Trumbull counties.

This per-visit role pays for each supervisory visit, open visit, skills test, and office time, with mileage reimbursed. Travel is required; hours vary as client volume grows over time.

Qualifications

• Must be 18 years or older.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver’s license, reliable transportation, and work-compliant auto insurance.
• Current CPR certification and ability to pass background checks and tests.

Responsibilities

• Documentation accurate and compliant with agency standards.
• Conduct regular supervisory visits every 60 days and open visits for new clients.
• Perform pre-employment testing for Home Health Aide candidates.
