Job summary

Oak Hill in Middletown, CT is seeking a Direct Support Professional for the evening, part-time shift to empower individuals with disabilities to live with independence and dignity in a residential setting.

You will deliver person-centered care, assist with daily living, monitor health, document in the electronic health record, and support community participation, while following training, safety protocols, and organizational values.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Commitment to maintaining a safe, healthy, and professional work environment.
• Willingness to stay beyond a scheduled shift when coverage is needed.

Responsibilities

• Assist with daily living activities including personal care, grooming, bathing, dressing, and hygiene needs.
• Monitor health-related information and document as required.
• Administer medications and maintain medication records per training and directives.
• Support health care plans and overall well-being.
• Transport individuals to appointments and community activities.
• Prepare meals and provide mealtime support according to menus and dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities and incidents in the EHR.
• Follow emergency procedures and safety protocols, and apply behavioral support strategies.
