Responsibilities

• Support individuals with daily living activities.
• Assist with personal care, grooming, hygiene, and mobility.
• Monitor health-related information and document it in the EHR.
• Transport individuals to appointments and activities.
• Provide nutrition and mealtime support and monitor safety.
