Job summary

Oak Hill in Mansfield, CT is seeking a Direct Support Professional for the day shift on a part-time basis. You will assist individuals with daily living, health monitoring, meal support, and mobility needs in a residential setting.

Responsibilities include documenting care in the electronic health record, following health plans, administering medications as trained, and promoting independence and community participation. A compassionate, reliable approach is essential to succeed in this role.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Assisting with personal care, grooming, bathing, dressing, and hygiene needs.
• Administering medications and maintaining accurate medication records (as trained).
• Monitoring health-related information and documenting progress.
• Transporting individuals to appointments and activities.
• Preparing meals and snacks according to menus and dietary needs.
• Assisting with mobility and transfers.
• Documenting daily activities in the EHR and following care plans.
• Following emergency procedures and safety protocols.
