Qualifications

• Commitment to maintaining a safe, healthy, professional work environment.

Responsibilities

• Support daily living activities and personal care per individual plans.
• Assist with continence care as needed.
• Encourage independence and skill development.
• Monitor health information and document medications as required.
• Transport to appointments and community activities using agency vehicles.
• Document in the electronic health record and follow regulatory requirements.
