Oak Hill is seeking a Direct Support Professional for the Evening Shift in Newington, CT. This part‑time DSP role focuses on person‑centered support in a residential setting, helping individuals live more independent lives with dignity and community connection.

You’ll provide daily living assistance, health monitoring, meals, mobility support, and transportation, while documenting in the EHR and following regulations.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.

Responsibilities

• Support daily living activities and personal care.
• Assist with continence care as needed.
• Encourage independence and skill development.
• Monitor health-related information and medications per training.
• Transport individuals to appointments and activities.
• Prepare meals and assist with mealtime support.
• Assist with mobility and transfers.
• Document daily activities and medication administration in the EHR.
• Follow regulatory requirements and emergency procedures.
• Communicate effectively with team members and stakeholders.
