Job summary

Oak Hill in Coventry, CT is seeking a Direct Support Professional for the evening shift to help individuals with intellectual and developmental disabilities live with independence, dignity, and community connections.

In this full-time role you will assist with daily living, health monitoring, medication administration, meals, mobility, and documentation, while staying aligned with our mission and policies. Schedule includes Tue-Fri 2pm-9pm and Sat 3pm-11pm, totaling about 36 hours weekly.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals.
• Ability to pass background checks and health screenings.
• Commitment to a safe, professional work environment.
• Occasional shift beyond schedule to cover needs.

Responsibilities

• Support individuals in healthy, meaningful daily living.
• Assist with personal care, hygiene, grooming and dressing.
• Administer medications per training and maintain records.
• Transport individuals to appointments and activities.
• Prepare meals and assist during meals according to needs.
• Assist mobility and transfers using approved equipment.
• Document daily activities and health information in EHR.
• Follow care plans and regulatory requirements.
• Collaborate with team and participate in trainings.
