Job summary

Trustbridge Hospice, a member of Empath Health, is seeking an organized and friendly Care Team Assistant in Fort Lauderdale. This onsite, full-time role supports the front desk and helps ensure a compassionate experience for patients and families during one of life's most meaningful transitions.

You will perform administrative duties, assist with patient charts, and help coordinate team meetings and care activities. Strong communication and computer skills with empathy and teamwork are essential.

Qualifications

• High school diploma or GED; 2+ years administrative experience in hospice or related field.
• Excellent verbal and written communication and solid customer service.
• Strong documentation, organization, time management and multi-tasking.
• Ability to work with staff, volunteers and multiple disciplines.
• Proficient in MS Word, Excel and Outlook; ability to learn new software.

Responsibilities

• Perform administrative clerical duties, file, enter documents, distribute mail and messages.
• Ensure patient charts information is accurate and share paperwork timely.
• Assist with weekly IDT meetings and coordinate events and trainings.
• Track team census by monitoring status changes and transfers.
• Support patient care needs and handle calls professionally.
• Communicate patient equipment needs to DME and maintain records.
