Job summary

Trustbridge Hospice, a member of Empath Health, seeks an organized and friendly Care Team Assistant in Fort Lauderdale. This onsite, entry-level, full-time role supports the Care team and helps ensure an empathetic experience for patients and families during life’s transitions, Monday through Friday 8:00 a.m.–4:30 p.m.

Responsibilities include administrative clerical duties, coordinating with various departments, tracking patient census, and assisting with IDT meetings.

Qualifications

• High school diploma or GED required; 2 years administrative experience in hospice or related field preferred.
• Strong communication skills, both verbal and written, and solid customer service ability.
• Excellent documentation and organizational skills, including time management, multitasking, and prioritization.
• Proficiency with MS Word, Excel and Outlook; ability to learn additional software as needed.
• Familiarity with medical terminology is a plus.

Responsibilities

• Perform administrative clerical duties (filing, entering timesheets, processing documents).
• Distribute mail, faxes, and phone messages promptly; maintain office supplies.
• Assist with patient chart accuracy and paperwork distribution across disciplines.
• Support IDT meetings and participate in agency events as assigned.
• Monitor patient census and track status changes in electronic systems; update care boards.

Trustbridge Hospice, a member of Empath Health, is seeking an organized and friendly Care Team Assistant in our team in Fort Lauderdale.This position is onsite. This entry level, full-time position works Monday through Friday, 8:00 a.m – 4:30 p.m., supporting the Care team and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.What You’ll DoPerform administrative clerical duties for team such as filing, processing/entering and copying documents (including timesheets for HHA’s, volunteer hours in Raiser’s Edge, etc.); distributing mail, faxes, and phone messages in a timely manner; maintain office and team supplies.Ensure information on patient charts is accurate.
