Job summary

Trustbridge Hospice, part of Empath Health, is seeking an organized Care Team Assistant in Boynton Beach, FL. This onsite, entry-level, full-time role supports the care teams and helps ensure a compassionate experience for patients and families during life’s transitions.

The schedule is Monday through Friday, 8:00 a.m.–4:30 p.m., with responsibilities including clerical duties, patient chart accuracy, coordinating information across disciplines, and assisting in IDT meetings.

Qualifications

• Entry-level administrative role supporting hospice care teams.
• Requires strong communication and organization.
• Experience in healthcare or hospice is a plus.

Responsibilities

• Perform clerical duties: filing, processing and copying documents.
• Ensure patient chart information is accurate and up-to-date.
• Distribute mail, faxes and messages promptly.
• Assist with weekly IDT meetings and related events.
• Track census changes and update records in electronic systems.
