Job summary

Trustbridge Hospice, a member of Empath Health, is seeking an organized Care Team Assistant for our Fort Lauderdale team. This entry-level, full-time role supports the front desk and ensures a seamless, empathetic experience for patients and families during life transitions.

You’ll perform administrative duties, manage records, and assist with team communications and census tracking. Onsite work schedule is Monday–Friday, 8:00 a.m.–4:30 p.m., with opportunities to grow in a mission-driven

Qualifications

  • High school diploma or GED required or equivalent
  • Minimum 2 years administrative experience in hospice or related field preferred
  • Excellent verbal and written communication skills
  • Solid customer service skills
  • Excellent documentation and organizational skills, including time management, multi-tasking and prioritizing
  • Demonstrated self-direction, independent judgment and discretion; able to collaborate with all team members and volunteers
  • Strong computer knowledge in MS Word, Excel and Outlook; willingness to learn additional software as needed
  • Medical terminology background preferred

Responsibilities

  • Perform administrative clerical duties (filing, processing/entering and copying documents, including timesheets for HHAs and volunteer hours)
  • Distribute mail, faxes and phone messages promptly
  • Maintain office and team supplies
  • Ensure accuracy of information on patient charts
  • Distribute paperwork to disciplines, departments and partners in a timely fashion
  • Collaborate with Team Leadership for weekly IDT meetings, supports and events
  • Track team census by monitoring status changes, levels of care, admissions, deaths, discharges and transfers
  • Update patient care board and census timely
  • Support coverage of patient care needs and handle calls professionally
  • Coordinate patient equipment needs with DME and maintain transaction records

Trustbridge Hospice, a member of Empath Health, is seeking an organized and friendly Care Team Assistant in our team in Fort Lauderdale.This position is onsite. This entry level, full-time position works Monday through Friday, 8:00 a.m – 4:30 p.m., supporting the front desk and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.What You’ll DoPerform administrative clerical duties for team such as filing, processing/entering and copying documents (including timesheets for HHA’s, volunteer hours in Raiser’s Edge, etc.); distributing mail, faxes, and phone messages in a timely manner; maintain office and team supplies.Ensure information on patient charts is accurate.
