The candidate will assess physiologic and psychosocial health, develop outcomes, formulate and implement plans, and provide education for patients and families, ensuring high-quality discharge

Qualifications

• Graduate from an accredited School of Nursing.
• Bachelor’s Degree in Nursing, preferred.
• Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
• Current license to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

Responsibilities

• Assess the patient’s physiologic health status and document plan of care.
• Assess the patient’s psychosocial health status and support needs.
• Identify patient outcomes and establish individualized goals.
• Formulate and prioritize plan of care, coordinating with physicians and care team.
• Implement the plan, monitor safety, and adjust as needed; document patient care and responses.
• Provide discharge instructions and coordinate referrals for post‑procedural care.
