Job summary

Prince George’s County Department of Family Services seeks a professional Support Planner to advocate, counsel, assess, and coordinate community-based social services for Medicaid Home and Community Based Programs. Responsibilities include education on Medicaid eligibility, enrollment assistance, housing referrals, and collaboration with local health and social service partners.

Qualifications

• Bachelor's Degree in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field.
• Responsible work experience which includes coordination of group activities, preferably with aging and disabled individuals.
• Must possess and maintain a valid Class C driver's license.

Responsibilities

• Provide program education on Medicaid eligibility and enrollment.
• Provide options counseling to nursing home residents and families.
• Provide ongoing case management and develop care plans using a person-centered approach.
• Conduct quarterly visits to monitor services and ensure plan of service adherence.
• Make referrals for community resources based on client needs.
• Assist with identifying affordable housing options and address related barriers.
• Collaborate with Health Department, Dept. of Social Services, MD Dept. of Health and other organizations.
• Maintain participants' information and case records in the program database.
