Job summary

Prince George's County Government seeks an entry-level Options Counselor (Community Developer 1G) in the Aging and Disabilities Division. The role provides information, counseling, and follow-up services to help individuals with disabilities and caregivers access Medicaid programs and community supports.

Under supervision, you will connect clients with program areas, gather documentation, and collaborate with local health and social service agencies.

Qualifications

• Bachelor's degree required in a related field.
• Experience coordinating group activities, preferably with aging/disabled individuals.
• Must possess and maintain a valid Class C driver's license.

Responsibilities

• Provide information, assistance, options counseling, and follow-up services to improve quality of life for individuals with disabilities and caregivers.
• Offer counseling on Medicaid Home and Community Based Programs and assistance with enrollment.
• Gather financial documents and submit CO Waiver applications.
• Provide ongoing case management including care plans using a person-centered approach.
• Conduct quarterly visits to monitor services and adherence to the plan of service.
• Refer to community resources and housing options as needed.
• Maintain participants' records in the program database and prepare reports.
