Job summary

Prince George’s County Department of Family Services seeks an entry level Support Planner to advocate, counsel, assess, and provide community social service assistance for Medicaid Home and Community Based Services programs and long-term care options. The role may require some evening or weekend work and is supervised by the Program Manager.

Responsibilities include program education on eligibility, enrollment assistance, and ongoing case management with care planning and referrals,

Qualifications

• Bachelor's Degree from an accredited college or university in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field.
• Responsible work experience which includes coordination of group activities, preferably including work with aging and disabled individuals.
• Must possess and maintain a valid Class C driver's license.

Responsibilities

• Advise, counsel, assess, develop, and provide community liaison/social service assistance for Medicaid Home and Community Based Services programs.
• Provide program education on Medicaid eligibility and enrollment processes.
• Provide options counseling to nursing home residents and families about community services.
• Perform ongoing case management including care plan development and monitoring progress.
