Qualifications

• Bachelor's Degree from an accredited college/university in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field.
• Responsible work experience which includes coordination of group activities, preferably including work with aging and disabled individuals.
• Must possess and maintain a valid Class C driver's license.

Responsibilities

• Provides professional social/work/case management services to individuals seeking enrollment in one of the Medicaid Home and Community Based Programs.
• Provides program education on the Medicaid programs eligibility, and application/enrollment process.
• Provides options counseling to nursing home residents and their guardians/family members interested in learning about services and support in the community related to Medicaid programs.
• Provides ongoing case management services including evaluation of needs, development of care plans, and monitoring.
• Establishes and maintains participants' information and case records in the program database.
