Job summary

Habitat Health seeks an Enrollment RN to serve as the clinical lead throughout the enrollment process for the PACE program. You will collaborate with the Outreach Specialist, Enrollment Specialist, and IDT to assess eligibility, educate participants and caregivers, and support informed enrollment decisions.

The role requires performing home safety and nursing assessments, NFLOC evaluations, and accurate documentation in the EHR.

Qualifications

• Associate's degree in Nursing required.
• Active, unrestricted RN license in the applicable state.
• Minimum of 1 year of nursing experience with older adults or complex medical needs.
• Experience conducting clinical and home safety assessments with sound clinical judgment.

Responsibilities

• Lead enrollment workflows with the IDT to ensure a smooth process.
• Conduct nursing, home safety, and eligibility assessments at participants' homes.
• Complete NFLOC assessments and related documentation per DHCS requirements.
• Present clinical findings in eligibility meetings to inform enrollment decisions.
• Build trust with participants and caregivers through education and a welcoming enrollment experience.
• Maintain accurate clinical records in the EHR and related systems.
